The mummified Charr in the desolation are probably from way before the Cataclysm. The reason being is that Joko was imprisoned and it was him who raised the army. So those Charr (and all other desolation undead) would be older then 200 years. Excluding the ghosts of course, as those are from Turai's pilgrimage.
I would have to say that those Charr are from before the humans came around, if they are the same Charr as the ones in the Charr Homelands/Ascalon. I would have to argue that they were some of the Charr that fought against the Forgotten, as according to the Ecology of the Charr, the Blazeridge Mountains were a constant battlefield, so it is possible they sent a few warbands as a guerilla warfare tactic (like the Ebon Vanguard) who were later raised as undead.
That's my theory at least (only other thing I can see would be Charr living/lived East of Ascalon/North of Elona).

That said, it's interesting that we don't see any undead Forgotten in Joko's army - or at least, not that I recall. Could that be because the Forgotten are somehow immune to being raised as undead, or was it that that was one fight Palawa decided not to pick?

Its probably due to him not being able to get to the Forgotton. Considering he hadnt expanded into the Crystal Desert and the Forgotton couldnt survive in the Desolation. Considering theres no mention of Joko before the War on Abaddon i assume he was not around at that time so by the time he did appear,with the exception of that lone witness, the Forgotton were probably long gone from the desolation.

Deep and Urgoz are dungeons and do not show up on the map, so it is hard to tell where they are exactly. That is, if you mean the fan-made picture . If you mean the in-game map, well, like I said, they are dungeons and just don't show up on the map. (Probably because of the old, must talk to x NPC to get in.)

The Deep is near Cavalon (Part of which can be partially seen on The Deeps map). Urgoz's map is darker so its hard to tell but i would assume it is connected to House Zu Heltzer. I would assume that the person who made the map thought the Deep and Warren were like the Realm of Torment.

There are Awakened in the Poisoned Outcrops, which is on the opposite side of Joko's Domain to the Lair of the Forgotten outpost. So it seems - to me at least - that the two did intersect.
That said, the description of the Lair indicates that it largely collapsed due to 'the expansion of the Realm of Torment' - it's possible that during Joko's first reign, it was inaccessible from the ground and Joko couldn't get in, while the Forgotten used it as an observation post.
